What to Expect At your First Physical Therapy Visit

If you've never seen a physical therapist before, or if it's been a while, you might be wondering what actually happens during that first appointment. As a cash-based physical therapist in Alexandria, Virginia, I want to walk you through exactly what to expect so you can show up prepared and get the most out of your time.

Before You Even Walk In: The Intake Form

Before your first visit, you'll receive a detailed intake form to complete. This covers your medical history, current medications, past surgeries or injuries, and a description of what's been bothering you. It might feel like a lot of paperwork, but the more thorough you are, the better. That information shapes everything that happens during your evaluation and helps me understand your full picture before we even meet.

We'll Talk In Depth

The first part of your evaluation is what we call the subjective intake, and it's one of the most important parts of the whole session. This is where I ask you about your symptoms in detail: when the pain started, what makes it better or worse, how it's affecting your daily life, your work, your workouts, your sleep. I'll also ask about your goals; not just "feel better," but what specifically you want to be able to do again.

This conversation takes time on purpose. Understanding your history and your life is what allows me to treat you as an individual rather than just a diagnosis.

Tests and Measures

After we talk, I'll do a hands-on assessment. Depending on what brings you in, this might include looking at your posture and movement patterns, assessing your range of motion, testing strength, checking how your joints move, and identifying where pain or restriction is coming from. For back and neck issues especially, this part of the evaluation gives me the information I need to connect your symptoms to their actual source (which isn't always where you feel it).

Your Treatment Plan

Once I have a clear picture of what's going on, we'll talk through what I found and what it means. I'll explain my clinical reasoning, what's driving your symptoms, what needs to change, and how we're going to get there. You'll leave with a real understanding of your body, not just a list of exercises.

Discussing Your Plan of Care and Scheduling

Because this is a cash-based practice, there are no insurance authorizations dictating how many visits you get or how often you come in. Your plan of care is based entirely on what you actually need.

We'll talk about visit frequency and duration, and you'll have the option to purchase individual sessions or a package of sessions at a reduced rate. Packages are a good fit if you're committed to a full course of care and want to invest in seeing it through. Individual sessions work well if you prefer flexibility. Either way, there's no pressure, just an honest conversation about what will get you the best outcome.

Setting Up Your First Treatment Session

Your evaluation and your first treatment session are typically scheduled separately. The evaluation is focused entirely on assessment and planning. Your first treatment session is where we start putting that plan into action with hands-on work, targeted exercises, and the beginning of real progress.

Knowing this ahead of time matters because some people come in expecting to feel dramatically different after the first appointment. The evaluation is about gathering information and building a roadmap. Treatment is where the work begins, and results build over time.
